Mouse droppings on syrup pumps, dead bugs on dishes close Philadelphia eateries

Philadelphia health inspectors closed at least 39 facilities for various violations from April 1 to April 15,

A live mouse scurrying around, slimy wall surfaces, dust and mouse droppings on condiments and cups filled with discarded cigarettes left in the dining room are all definitely not something you want to find if you are eating out.

But that is some of what Philadelphia Health inspectors encountered this month.

There were also some familiar names in this edition of Clean Plates including two different Jimmy John’s Gourmet Sandwich locations, Home Depot, a Federal Donuts, a Dunkin Donuts, a Boston Market and a Domino’s Pizza.

From April 1 to April 15, inspectors closed at least 35 facilities for various violations.

Inspectors visit nearly 1,000 eateries and food retailers every two weeks. Each inspection is generally regarded as a snapshot in time, and not necessarily a reflection of day-to-day conditions. Most violations were corrected immediately in the presence of an inspector.

A list of eateries that were closed follows:

642 N Broad St.

20 violations, 8 serious

Running water was not available at the warewash and food prep sinks; a food employee was seen touching ready-to-eat peppers and onions with their bare hands; an employee was rinsing a knife in the handwashing sink instead of properly cleaning and sanitizing in the warewashing sink; a container of cut lettuce was uncovered in the prep unit refrigerator; cooked chicken, pasta, and boiled eggs were not properly cooled prior to mixing with cold foods; salads, cut lettuce, and sliced tomatoes were not held at the required temperature; employee medicines were stored in counter area, and were not stored to prevent the contamination of food, equipment or utensils; the oven was not under an exhaust hood; there were leaks from the valves under the warewashing sink.

The establishment agreed to discontinue food operations and voluntarily close until it is approved by the Department to resume operations. Inspected April 15.

1603 W Erie Ave.

20 violations, 7 serious

Food employees were eating in the food prep area; chicken, shrimp, egg rolls were stored uncovered in the under counter refrigerator and the walk-in cooler; chicken, burgers, cheese, and shrimp food were not held at required temperatures; employee medicines and personal hygiene products including toothbrush and toothpaste were stored in the food prep area, and were not stored to prevent the contamination of food, equipment or utensils; shrimp was being thawed using an unapproved method; grease accumulation and heavy food residue and debris were on the wok station and on the cold holding equipment at time of inspection; cutting boards had deep scratches and scoring not allowing for effective cleaning and sanitizing; there was visible physical evidence of rodent/insect activity in the food prep, warewashing, and basement storage area, the rear outer opening in the food facility did not protect against the entry of insects, rodents, and other animals.

The establishment agreed to discontinue food operations and voluntarily close until it is approved by the Department to resume operations. Inspected April 15.

Nori Sushi

1636 South St.

A dead mouse was found on a sticky trap; old and new mouse droppings were found throughout the facility including in the basement, on shelving units where retail items are stored, on a slicer, under bags of onions, around the water heater and under the stairs; there were dead roaches found on sticky traps; puddles of water were found around the water heater in the basement; there was food debris under and around equipment in the kitchen; old food residue and containers were in the handwash sink, in the sushi preparation area; the handsink in the basement had plastic bags and container; food was not stored at least six inches above the floor, in the walk in freezer; rags were drying above the three compartment sink; utensils were soaking in bucket of cloudy water.

The establishment was issued a Cease Operations Order and must discontinue food operations immediately. The establishment could not operate for a minimum of 48 hours. Inspected April 15.

208 S 13th St.

16 violations, 6 serious

The soda gun holsters at all bars had an appearance of residue at the bottoms; handsinks at the bar area were being used as a dump sink; a person in charge was not present at the food facility during all hours of operation; shellstock located in the kitchen area did not have identification tags attached to the container; all doors and windows were open during time of inspection; the food facility was preparing foods using reduced oxygen packaging without the required written procedures or food safety plan; the person in charge of premises has not posted the appropriate warning signs regarding the dangers of consuming alcoholic beverages during pregnancy, to remind food employees to wash their hands, and any “No Smoking” signs.

The establishment agreed to discontinue food operations and voluntarily close until it is approved by the Department to resume operations. Inspected April 13.

1923 Fairmount Ave Unit C-3

33 violations, 13 serious

There was no hot water; cigarettes, cigarette butts and cups with cigarettes were found in the dining, storage, and basement prep areas; the handwashing sink in the cook line area was blocked by containers with rags, and tongs observed stored on the sink basin; not accessible at all times for employees; food debris was found inside of the handwashing sink in the warewashing area; the microwave cavities had food debris on the surfaces; the food safety certified person was not initially present; the food safety certified person could not ensuring that food employees were aware of proper warewashing procedures; bean sprouts and cooked noodles were not stored at proper temperature; there was debris on the floors and walls in the food prep, warewashing, and storage areas; there were damaged wall and ceiling surfaces in the basement; the slicer had an accumulation of food residue on the food contact surfaces and was not being washed, rinsed and sanitized at least every four hours; the can opener had an accumulation of food residue on the food contact surfaces; the interior surfaces of the ice machine was unclean and had pink and black residue observed on contact surfaces.

The establishment agreed to discontinue food operations and voluntarily close until it is approved by the Department to resume operations. Inspected April 15.

248 South St.

15 violations, 5 serious

Mouse droppings were found in containers with clean dishware and equipment; on shelving units, on containers of food, in lighting fixture, under and around equipment along the perimeter of both upstairs kitchen and in the basement and in the storage closet; dead flies and other debris were found in the bottom of the Coca-Cola refrigerator; some dishware had a greasy film and small dead bugs, a shelf that dishware was stored on had an accumulation of grease; there were small flies in the basement by the handsink, a house fly was in the employee toilet room; food debris was found in the walk in box, and on the walls, under shelving and under the sinks in the basement; black residue was on the interior of the ice machine bin; there was a domestic microwave, coffee pot, mini refrigerator and deep reach in freezer chest; food employees were donning single use gloves without a prior hand wash; the handwash sink in the basement food preparation area was blocked by a pot and not accessible at all times for employee use; cutting boards had deep scratches and scoring not allowing for effective cleaning and sanitizing; there was clutter in the closet.

The establishment was issued a Cease Operations Order and must discontinue food operations immediately. The establishment could not operate for a minimum of 48 hours. Inspected April 12.

1800 S 4th St.

20 violations, 6 serious

Adequate hot water is not provided throughout the facility; the paper towel dispenser at the handwash sink in the food preparation area was empty; containers of sour cream, cheese, baking soda, and others were reused for food storage; the food preparation area refrigeration unit condensate line observed draining into a plastic container; a food employee was washing dishes without rinsing prior to sanitizing; the person in charge did not demonstrate adequate knowledge of the PA Food Code; there were uncovered foods observed in several refrigeration units and freezers; potentially hazardous ready-to-eat food, prepared in the food facility and held for more than 48 hours, located in a food preparation area refrigeration unit, were not date marked; utensils were stored in a cup of stagnant water; bulk ingredient scoop handles were in direct contact with bulk food ingredients there was grindable food waste in food preparation area trash cans.

The establishment agreed to discontinue food operations and voluntarily close until it is approved by the Department to resume operations. Inspected April 12.

3400 W Girard Ave.

6 violations, 1 serious

Hot water was not available at handwash sink; there were rodent droppings behind ice cream chest freezer; ice dispensing utensil in the ice bin were in direct contact with ice and did not have the handle stored above the top of the food; food debris accumulation in ice cream chest freezer; dirt and debris accumulation was found along floor perimeters throughout the facility.

Roaches were along the seam/gasket and under the freezer in the food prep area; general cleaning needed for the dust and food debris under the equipment in the prep area; old mouse droppings were on lower shelving in prep area; containers in the food prep area were not inverted to prevent contamination; soda crates were being used for elevation in the rear area and cardboard lining surfaces of equipment were not smooth and easily cleanable; there was water damaged ceiling tiles in the food prep area; there were damaged wall areas in the basement

The establishment was re-issued a Cease Operations Order and must discontinue food operations immediately. The establishment could not operate for a minimum of 48 hours. Inspected April 11.

2001 N 13th St.

11 violations, 8 serious

Hot water is not provide; food items in the ice bin were not held at the proper temperature;

The food safety certified person did not have photo identification present to verify identity; the vendor did not have an approved commissary; cooked foods in vacuum sealed bags were not being held at proper temperature; the food items including cooked rice, meat and beans were being sealed in bags without any prior plan submission for a food safety plans to the health department; the mechanical exhaust hood was not sealed to the ceiling of the truck; foods vacuum sealed in bags did not have labels including all information necessary as per food safety guidelines.

3700 Sansom St.

There was no hot water; the food Employee did not follow proper handwashing procedure, soap and/or warm water were not used and they did not wash hands between changing tasks; food was being stored inside of plastic shopping bags; food inside of the refrigeration unit was uncovered; a general cleaning was needed; moist wiping cloths were lying on work surfaces and not stored in sanitizing solution.

Condensation wastewater from the upper level drop bin was draining into the cooler beneath it where food and ice were stored, the food was not protected from potential contamination; drop bins where food was stored with ice, and storage areas were unclean with stagnant water and food debris; rice noodles, cooked rice, shrimp, chicken, cooked mixed vegetables, and tofu were not held at a proper temperature; floors and wall surfaces had grease and other debris on them; the exhaust hood filters and faucet handles were greasy and unclean; sliding windows were open without a protective barrier or screen and not protecting against the entry of insects, rodents, and other animals; there were open employee beverage containers; a food safety manager or food safety certified person was not present; an adequate amount of paper towels were not available at the handwash sink; food employees were not wearing or wearing ineffective hair restraints, such as nets, hats, or beard restraints.

There was no running water on site; bacon slices were not held at proper temperatures; food debris was on the deli slicer deflector arm, blade and plate; raw chicken was stored above raw beef steaks and patties, and fries; deli meats and cheeses were not properly date marked; there was dust on the mustard bottles in the retail area; the handles of equipment including the refrigerators were not clean to the touch; grease was on the floor by the fryer; dishes and other unclean wares inside the hand wash sink; soap and paper towels were not present at the hand wash station in the food preparation area; sugar and food debris were found on the coffee counter and deli slicer table; there was grease accumulation in the fryer; an unapproved microwave was in the food preparation area.

2200 W Oregon Ave.

5 violations, 2 serious

Prepackaged candy and chocolate bars had been ripped and destroyed by rodents; mouse droppings observed in prepackaged storage containers; there was visible physical evidence of rodent activity in the retail area/at the cash registers; fresh and old mouse droppings were found on all floors throughout the facility.

1406 Spring Garden St.

16 violations, 6 serious

Dried spinach leaves and other debris were on the floors of the truck; mold, mildew, and slime were along the wall surfaces by the handwashing sink; the facility was not approved to prepare green smoothies containing leafy greens and/or melons such as honeydew, watermelon, and cantaloupe; the melons were not held at proper temperatures; the food safety certified person did not have an adequate amount of multi-use utensils, only one spoon was available; there were holes in the floors, walls, and rear doors of the food truck; food employees were donning single use gloves without a prior hand wash; ready-to-eat foods fruit were stored in non-food grade plastic containers and bags; there was unapproved equipment in use including a refrigerator-freezer unit, colanders, storage containers, and chest coolers; the food safety certified person was not able to explain how to properly wash, rinse, and sanitize multi-use dishes and utensils at the commissary; the front sliding windows were open without a protective screen/barrier; not protecting against the entry of insects, rodents, and other animals; alteration or construction began prior to plan submission and approval-menu change.

Eggs were stored above ready-to-eat foods in the back refrigerator; cut tomatoes, cut cucumbers and creamed cheese were not held at the proper temperature; a bottle of cleaner was stored above or on the same shelf with food, equipment, and/or single service articles in the basement storage area; there were uncovered chicken and potatoes in refrigeration units in the basement storage area; there was debris and residue on interior surfaces of refrigeration/freezer units, second floor server station, and surfaces of basement work table; there was unapproved domestic equipment including one mini refrigerator on the second floor; a refrigerator and three freezers in the basement; there were unapproved shelves observed above the three-bay sink; there was a defective mini refrigerator; debris was found on floors under/around/behind equipment along floor-wall juncture and perimeter in the food prep areas, basement, warewash area and second floor; there was unnecessary clutter in the basement; there unsealed utility/plumbing lines throughout facility.

A dead roach was on the lid of canned milk and dead roaches were found on retail area shelves; roaches were in the storage area; flies were found in the retail area; there was a cat on the premises; there was expired milk and severely dented, swollen, distressed canned items on the retail area shelves; the rear door of the facility does not protect against the entry of insects, rodents, and other animals and the front door of the facility remained opened; domestic shelving were found; there was a defective display case in the retail area; there was unnecessary clutter and equipment in the rear storage area and behind the cash register; water damaged and missing ceiling tiles were found in the rear storage and restroom area; there were damaged floor tiles throughout the facility.

2025 S Broad St.

10 violations, 5 serious

Mouse droppings were on syrup pumps and boxes of tea bags in the front food preparation area, on prep tables and lower food prep table shelves, inside a box of disposable cups, and along floor perimeters in the front and rear in the front food preparation area; dead roaches observed on the floor in the storage area and in the front food preparation area near the mop sink; eggs were held at an improper temperature; soap was not available at the handwash sink in the restroom; a spoon was stored in a cup of stagnant water; the front food preparation area refrigeration unit was not able to maintain a proper temperature; there was an unused ventilation hood in the rear food preparation area.

6649 Germantown Ave.

3 violations, 2 serious

The food safety certified person arrived 20 mins after start of inspection; chlorine sanitizer solution was being used in excess; the food facility lacks chlorine sanitizer test strips to monitor sanitizer concentration.

Water ice was being dispensed in the retail area; a food employee did not wash hands between changing tasks; an employee working front collecting money, then proceeded to dispense water ice in retail area; the food safety certified person was not present; the establishment was continuing to cook unapproved hot foods via griddle and fryers but did not have adequate ventilation; a mechanical hood system was lacking; deli meats and cheeses; opened commercially processed ready-to-eat food, located in the display case and held more than 48 hours, was not marked with the date it was opened; food employees were not wearing or wearing ineffective hair restraints, such as nets, hats, or beard restraints; domestic unapproved deep freezer and college style refrigeration unit were in use; the garbage disposal was defective; a backflow prevention device was not on the water line near water meter.

Mouse feces were observed on the floor and the lower surfaces in the basement dry storage area.; there was dust, debris, residue, and mouse feces in the food preparation areas, warewashing area, basement storage area on the floors and around equipment; there were expired 32 oz. yogurt containers in the front display case, black residue was found on the interior of the ice machine bin around the upper rim located in the basement storage area; the handwashing sink located in the food preparation area was not properly sealed to the adjoining wall with an approved caulk; there was unused equipment in the basement dry storage area; an unused three-compartment sink was in the rear food preparation area; the door of the reach-in freezer located in the basement dry storage area was not in good repair; brooms and dust pans were being stored on the floor in the food preparation area and basement storage area; alteration or construction began prior to plan submission and approval.

Running water was not provided in the truck for employees to wash their hands; utensils were being washed in the hand sink and stored on the drying rack without being sanitized; grease residue and droplets were present on the shelf above the small wok station, which did not have mechanical ventilation overhead to help prevent contamination; the thermometer in the bain marie refrigerator was inaccurate; cardboard was used to line floors on the truck; the mechanical exhaust hood was equipped with residential style fiberglass HVAC filter cartridge, not commercial grade, and is not fire-proof; the cutting boards had deep scratches and scoring that did not allow for effective cleaning and sanitizing.

The food prep sink cold water faucet was leaking and being regulated with the shutoff valve underneath the sink; the food safety certified person was not present; debris was found along floor perimeters of the walk-in cooler under the shelving; there was no chemical sanitizer, food-grade chlorine bleach or required ammonium compound tablets, for the sanitization of utensils and food equipment; static dust had accumulated on the ceiling and wall air conditioning and heating ventilation covers in rear food storage area.

Soap was not available at the handwash sink in the toilet area; there was no sign or poster at the handwash sink in the toilet area to remind food employees to wash their hands; the handwash sink in the toilet area does not have single use towels, continuous towels, or air drying device; dust was on shelving behind service counter; maintenance tools directly on floor surfaces and not elevated on utility rack when not in-use; water stained/missing ceiling tiles were found; a change of ownership application had not yet been received.

A live mouse was seen coming up from the basement then hid behind door to basement and ran somewhere in the facility; mouse droppings were found on the basement steps and through out the basement; the sink in the employee toilet room was broken and not in working order; a food employee was seen washing their hands at the three-compartment sink rather than the designated handwash sink; there was no soap and paper towels at the handsink adjacent to the three-compartment sink and employee toilet room; rags were stored in the handsinks; a person in charge was not present at the food facility during all hours of operation; insecticides or rodenticides were not labeled by the manufacturer for food facility usage; there was a leak at the broken handsink in the employee toilet room.

Potentially hazardous ready-to-eat food, such as deli meats, cheese opened in the food facility and held for more than 48 hours, located in the deli case were not date marked; several food items including deli meats and cheeses in the display case were not held at the proper temperature; the interior of reach-in freezer and deep freezer had food debris and ice accumulation; cardboard, which is not easily cleanable, was lining shelving; there was no thermometer in refrigeration unit in basement; food employees were not wearing or wearing ineffective hair restraints, such as nets, hats, or beard restraints; there was a hole in the floor by warewashing sink; the toilet room door was not self closing; a non-food grade black garbage bag was used to store ready-to-eat long rolls; there were opened cans of mushrooms.

Hot water was not available at the handwash sink on the mobile unit, which also had inadequate water pressure; raw fish, cut lettuce, and sliced tomatoes were not held at the proper temperature; a food safety certified person was cutting leafy greens on the mobile unit which is prohibited; the exhaust hood was not properly sealed along the edges; the windows of mobile unit did not have protective screening and did not protected against the entry of insects, rodents, and other animals; food employee observed eating food on the mobile unit; food employees observed not wearing or wearing ineffective hair restraints, such as nets, hats, or beard restraints.

9813 Bustleton Ave.

There was visible physical evidence of rodent activity in the basement area; severely dented, swollen, distressed canned items observed in basement storage shelves area; a bowl of cooked meat potentially hazardous ready-to-eat food, prepared in the food facility and held for more than 48 hours, located in the walk-in cooler, was not date marked; food employees were not wearing or wearing ineffective hair restraints, such as nets, hats, or beard restraints; moist wiping cloths were lying on prep table and not stored in sanitizing solution; a person in charge was not present at the food facility during all hours of operation; there were cutting boards with deep scratches and scoring that did not allow for effective cleaning and sanitizing; brooms were lying on the floors in the basement area and not hanged or properly stored to prevent vermin harborage.

7240 Torresdale Ave.

7 violations, 2 serious

Damp soiled cloths in an open basket near the warewash sink were attracting flies, mouse droppings were found in the food prep, warewashing, and cooking areas; there was a fruit fly infestation in the food prep, warewashing and storage areas; food including chicken was not protected from contamination in the walk-in refrigerator; a bucket containing grease residue was open in the rear warewashing area; prepackaged desserts on the front counter were not labeled properly with the name of product, ingredient statement, net weight, distributed by statement and/or nutritional facts; the paper towel dispensers in the warewash and women’s toilet room were empty.

5143 Baltimore Ave.

9 violations, 4 serious

A food employee was observed touching cheese with his bare hands; there was uncovered cooked home fries under flat top grill, unwrapped cheese and uncovered peppers and onions in the refrigerator; the food employee did not follow proper handwash procedures; pots and dish of food including collard greens and shrimp were not stored at least six inches above the floor; a domestic microwave was found; there was an accumulation of grease and food debris on the outer sides of a flat top grill and deep fryer; the toilet room was in need of cleaning.

There was expired baby food observed in retail area; cut lettuce and tomatoes were found in the facility that does not have an approved preparation sink; the handwash sink in the food prep area was blocked by food equipment and not accessible at all times for employee use; the three-basin warewash sink was disconnected and inoperable; slicer has an accumulation of food residue on the food contact surfaces and was not being washed, rinsed and sanitized at least every four hours; deli meats and cheeses were opened commercially processed ready-to-eat food, located in the display case, and held more than 48 hours, were not marked with the date they were opened; a chest freezer, coffee maker, counter top mini deep fryer on microwave, soda crates used for elevation and toaster were not approved commercial equipment; the garbage grinder was attached under a wall mounted handwash sink; the type of kitchen handwash sink was not approved; there was cracked glass on a display case in the food preparation area and on the front door; wood shelves in the retail area and storage area were not smooth and easily cleanable; aluminum foil was used as shelf liners in the food preparation area; an unused chest freezer was in the retail area; there was a broken handle on “Canada Dry” refrigerator; the walls throughout the facility were damaged.

The establishment was issued a Cease Operations Order and must discontinue food operations immediately. The establishment could not operate for a minimum of 48 hours. Inspected April 1.

There were numerous dead roaches in bottom of the cold unit where open food was found; old and new mouse feces were on the slicer, and on counter behind the slicer, on a counter with bags of bread and candy display area; roach eggs were inside the seam of stainless steel counter coffee prep area; there was heavy visible physical evidence of insect and mouse droppings in the food prep, warewashing, rear storage area and in basement storage area; the shelves and crates needed cleaning needed due to dead roaches, roach droppings, debris and mouse feces; a can of roach spray was found in the prep area; there was open bait on the floor in food prep and storage; grease accumulation was found on sides of food equipment; pipes and utility lines were not sealed; there were holes and gaps in the walls in storage area; there was heavy ice accumulation in the reach-in freezer and deep freezer unit.

The establishment was issued a Cease Operations Order and must discontinue food operations immediately. The establishment could not operate for a minimum of 48 hours. Inspected April 1.
