Text of statement

Crozer Keystone Health System issued the following statement Wednesday  morning in response to questions about a state inspection report that identified deficiencies involved with use of an endoscope:

As part of its commitment to being a High Reliability Organization, Crozer-Keystone encourages the immediate reporting of potential safety risks that may occur during patient care. As such, in this case, the health system became aware that an employee may not have completely followed protocols related to the high-level disinfection of an instrument used in a procedure at Crozer-Chester Medical Center. We immediately reported this situation to the Department of Health and took numerous actions to ensure the safety of our patients and to provide employees with the necessary re-training.

We fully participated in a visit by the Department of Health and submitted a comprehensive plan of correction. We are awaiting a response to this plan. We are also in discussions with DOH officials regarding our concerns about the accuracy of some of the dates and information in the published report.

We are proud of Crozer-Keystone Health System's commitment to providing high quality care, which we demonstrated by our swift and comprehensive response to this situation. We regret that we cannot speak about the status of the patient involved in this situation, but privacy laws prohibit us from doing so.
